Everyone retires to bed and eventually get to sleep, despite the disturbing animalistic cries that seem to be coming from outside. The characters again find themselves waking up in the dream hospital. Everyone meets in the corridor, including Dan, who looks even worse now but wants to come with them. They agree to let him and set off down the corridor towards the stairs.
They come Roman seated at a large table. He invites them to sit with him and have a civilised discussion before tomorrow, when “the true game begins”. People arm themselves with cutlery from his table. Seeing this he laughs and offers them a gun which Christine accepts. He is asked about his murders and tells them that his victims were of no consequence. Kevin asks him what gives someone consequence, Roman replying he is the only one of any consequence, because he created himself. “In the beginning was the Word, and I was the Word, and the Word was with Me”. Roman advises them that they should leave the hospital tomorrow. He will arrange for the gates to be left open and unguarded during Nurse Shields funeral tomorrow. If they stay, he warns them things will get scary and he will kill them next time he sees them. He then leaves.
Everyone heads downstairs into the entrance foyer where they meet Cameron Mueller, the Toxteth Cannibal, who is busy eating an orderly. He sees them and they have a conversation where he advises them not to go outside, because “it hurts”. Samuel gets freaked out when Cameron starts eyeing him up as a potential meal, and eats part of the dead orderly that is offered to him. He throws up violently and Cameron looks disappointed before going back to his meal. He runs upstairs to rinse his mouth out with the remains of Roman’s banquet but finds everything he consumes turns to blood in his mouth. Still consumed with guilt he returns to the foyer where the group open a door to the outside. The outer surface of the door (carved with the depiction of a saint slaying a dragon) is glowing slightly. From the darkness outside a butterfly flies in, crosses the foyer and goes down the liftshaft. The group call the lift and follow.
The lift opens opens on the long T shaped corridor where they previously met Roman. The inspect his room, which is empty, but covered in occult symbols. As they move down the corridor a voice calls out to Samuel, seeming to recognise him and asking for help. He claims to be Steve Maxwell, an orderly Samuel made friends with on his initial incarceration, but he looks nothing like him, and the name on his door is ‘John Smith’. He claims to not know why he has been locked up and seems to be confused as to why they are freely wandering around. Samuel questions him, and he seems to have knowledge that only Steve and himself would know so he decides to free him. ‘Steve’ is very grateful and tells them that keys for the maximum security ward are kept in the governess’s office, if they can find a way into there.
They press on, accompanied by ‘Steve’, through the room with the large engraved face and down the stairs into the room below. Here they see a figure Christine recognises leaning over an operating table, performing surgery on something. Noises can be heard from the dark recesses of the room. In the centre of the pool sits the butterfly. As they approach the surgeon looks up and laughs at them – an overweight balding man, with folds of loose green skin intershot with thick black veins and yellow cats eyes. He unleashes his new creation on them – a creature with no face, but extra limbs and mouths surgically attached. As it lumbers towards them, more creature move out of the shadows all around. The butterfly is swallowed by a huge creature – far bigger than the pool would seem to hold. As the group begin to panic, ‘Steve’ falls to one knee, holding his head, and in Andreas’s voice calls out “Samuel, do what I told you for once and follow the damned butterfly!” As the abominations close in on them the group leap into the pool, plunging into the dark depths, where they are swallowed by a huge creature. In the belly of the beast they see the butterfly, partially digested but still alive. Samuel remembers Andreas’s instructions and visualises the door from his childhood, and it materialises before him.
Through the door they find themselves in Samuel’s kitchen from his childhood, Bes is seated at the dining table, leaning back on a chair, dressed in circus gear and face paint depicting a clown on one side, a devil on the other. She eyes the group suspiciously. Andreas, dresses as a circus ring master, welcomes them and invites them to sit. He asks Gabriel to keep an eye out of the window whilst he talks to the group. He tells them he comes from a line of wandering people who’s ancestors were assigned to guard the seed of the tree of life. He tells them the tree is under threat. He tells a story about a land he once saw who’s people were ruled by a fictional king. The best artists in the land were recruited to communicate with the king. He explains that imagination and reality are but different branches on the same tree. The tree is now under threat and the task of saving has fallen to the people here. Andreas doubts their ability to succeed as all show signs of corruption (which seems to rile Kevin) and Andreas must now flee as Samuel has betrayed his position to the enemy. At this point Gabriel spots figures starting to appear on the horizon, just a few at first, but very soon the hillside is blanketed with an incoming horde.
Andreas hurriedly continues to explain that the asylum is on a place of power, a nexus. Some say an ancient God slumbers under the hillside the asylum is constructed on. Four centuries ago a sorcerer came to this land and struck a dark barging to access this power. He has corrupted the power of the area, twisting it and turning it into the labyrinth – a place that not only traps souls, but joins them together, providing brave souls with perhaps the opportunity to penetrate the treasures within. Roman Carlisle now stands at the other end of this 400 year plan, working with the dark spirits that have been corrupted by the sorcerer. Soon they will begin to break open the seals and when the final seal is broken they will be unstoppable. Andreas warns them that Roman and the spirits are immensely powerful, and if they focus on trying to stop them this will just create more resistance and make them more powerful. They all walk a fine line between damnation and salvation and should choose their paths carefully.
With this the horde arrive – an army of dessicated walking dead, the air is filled with the noise of their whispering, a gentle sinister roar, which promises oblivion to those who listen too closely. Andreas and Bes realise they can’t wake up and realise they are all trapped in the dream. He shouts at everyone to defend the entrances as the zombies begin to break through the doors and windows and tells Samuel he must open a door.
Samuel eventually realises what he must do and visualises a door back to his room in the asylum and everyone flees. Here Andreas has time to apologise to Samuel, saying his soul has become a spiritual battle ground, before zombies start to appear outside. Samuel opens another door this time to his wedding day and everyone flees.
In the church Samuel sees his wife Alice waiting at the alter, she looks confused and asks him why he is here. As they speak the door open and a man enters that everyone recognises from their dreams, the man they saw standing in front of the asylum burning crosses, before they were committed. He walks down the isle as the church organ music plays, smiling at the group, emanating a dark aura of energy. He kisses Alice and melts into her, becoming a dark mark over her lungs (where Samuel knows her cancer originated), a pair of lips that mock Samuel and the group as more zombies appear and the congregation screams and panics. Christine and Andreas seem to see something that no one else spots, Andreas says Death is coming, but it may give them a chance to escape – if someone dies it will provide a window of opportunity for the group to wake up. He goes and talks to Bes and hands her something, telling her to look after it. She screams at him at he walks forwards into the oncoming horde and his torn apart into nothing. Bes screams curses at the group as she wakes up. Samuel and Gabriel manage to wake up, Dan, Kevin and Christine struggle to wake, Kevin and Christine both grappled by zombies. Daniel saves Kevin, pushing him free of the zombies and both manage to successfully awaken.
Back in the hospital everyone meets in the corridor. Christine is missing. They look in her room and she is still in bed. Gabriel checks and there is no pulse, they shout for help and look on helpless as a resuscitation crew arrive. They are ushered out of the room. Minutes pass and eventually a trolley is wheeled out, a blanket covering a still and lifeless body, which is wheeled away.
